Roundtree later married Karen M. Ciernia in 1980; they had three children together before divorcing in 1998. In 1993, Roundtree was diagnosed with breast cancer, and subsequently underwent a double mastectomy and chemotherapy. Roundtree died of pancreatic cancer at his home in Los Angeles on October 24, 2023, at the age of 81.
