The lightning bolt impacts the corpse of Kos, and the middle wave will always track your position. Obviously, if you're far out into the water, it will be very easy to dodge the wave of lightning. This means that the few seconds of Orphan screaming is basically free damage, making that attack stop being dangerous and start being an excellent time for healing and/or damage.
