The duchess in question was Elizabeth Howard (d. 30 November 1558), eldest daughter of Edward Stafford, 3rd Duke of Buckingham, and Eleanor Percy.

Elizabeth Boleyn, Countess of Wiltshire, born around 1480 Elizabeth Howard, was the daughter of Thomas Howard, earl of Surrey. She was the eldest of the two daughters of Thomas Howard and Elizabeth Tilney and was descended from King Edward I (Fraser, pg. 116).
Elizabeth Boleyn, born Elizabeth Howard around 1480, was an English noblewoman and the mother of Anne Boleyn, who became Queen of England, and thus the maternal grandmother of Elizabeth I of England. She married Thomas Boleyn, and through this union, she was styled Viscountess Rochford, Countess of Ormond, and Countess of Wiltshire.
She was a maid of honour and lady in waiting to Queen Elizabeth I, as was her sister Frances Howard, Countess of Kildare . She married Sir Robert Southwell (1563—12 October 1598) of Woodrising, Norfolk, on 17 April 1583. He was the son of Sir Thomas Southwell and his second wife Mary Mansell, a daughter of Sir Rice Mansell (1487–1559).
