A new classification system for mullerian anomalies based on the iconic AFS classi cation from 1988, but expanded to include all categories of anomalies. The goal is to raise awareness, educate providers and learners, and promote patient advocacy for these rare and complex conditions.
The ASRM Müllerian Anomalies classification 2021 (MAC2021) classifies müllerian anomalies into the nine categories below. Because müllerian anomalies represent a continuum of development and many have combined elements, some anomalies appear in more than one category.
There are many proposed classification systems for müllerian anomalies. The American Fertility Society (AFS) Classification from 1988 has been the most recognized and utilized. The advantages of this iconic classification include its simplicity, recognizability, and correlation with clinical pregnancy outcomes.

Here is the challenge: the müllerian anomalies each represent defects of embryonic development and have been shown to occur at any site in the process. The possible separate defects are numerous, and many of them have yet to be identified. Let us put this in perspective by pausing and thinking about reproductive development.

The ASRM Müllerian Anomaly Classification 2021 (MAC2021) was created with several goals in mind: Build on the known strengths, simplicity, and recognizability of the 1988 AFS Classification. Update and expand the classification to include cervical and vaginal anomalies utilizing an interactive format.
