20/20 is a passive item. Isaac fires two tears at once, one from each eye. 0.8x damage multiplier. Unlike other multiple shot items like Mutant Spider or The Inner Eye, 20/20 does not reduce the rate of fire.

Purist is challenge #20. This challenge is unique because Isaac starts with no items. The goal is to defeat Mom's Heart or It Lives. This Challenge is unlocked after defeating Mom for the first time.
Epic Fetus is an unlockable passive item. Isaac's tears are replaced with guided missiles. Pressing the fire buttons spawns a crosshair that can be moved freely, summoning a missile to its position from above 1.5 seconds later. Missiles deal 20 times Isaac's damage.
20/20 doubles lasers without increasing tear delay. Abaddon Aside from the lack of Angel Rooms, it provides no downside. As it costs 3 Soul Hearts, it will add 3 Black Hearts to his health bar, increase damage and speed, and give his Brimstone laser a fear effect.
