You do this in the Acherus Hold at the Runeforges. Death Gate-Summons a gate that will teleport you to Acherus.

The Unholy opener contains a ton of different abilities you need to press, making the use of macros a good way of simplifying it. Most trinkets are 20 seconds duration, meaning that they fit in nicely with Unholy Assault in the opener: / use Unholy Assault. / use Empower Rune Weapon.
