Kaoru Hanayama is one of my favorite characters. His fight with Speck was one of the best in the show and the injuries to his face were metal. That said, it makes little sense to me that that seems to be one of the only permanent injuries suffered by a main or side character. The only other thing I can think of is Dorian's ear.

Description. Thanks to his superhuman grip strength, Hanayama can do great damage to his opponents. During this technique, he grabs his opponent's body parts (such as arms, or legs), and then he holds it so tight that it explodes due to the blood restriction, making that body part useless.
Description. Kaoru Hanayama can perform an incredibly strong punch known as Destructive Force. He uses a combination of his supernatural skills, which consist of grip, body weight and speed, he then proceeds to punch his opponent.
