Juggernaut Drive appears very similar to the Scale Mailarmor with a much more Dragon-like appearance, in the anime it takes a more gigantic form. The armor possesses claw-like growths on the hands and feet, as well as horns and a fanged mouth on the helmet.

Not necessarily a transformation, but I thought the perfected juggernaut drive just referred to the ability to use juggernaut drive without losing your sanity, and thus you can fight in the form with normal consciousness (at the expenditure of your mana and/or lifespan).
