Im April 1912 vertraute die Reederei ihm das Kommando über die Titanic an, die noch auf der Jungfernfahrt gegen einen Eisberg stieß.

Smith var anställd av White Star Line och var befälhavare på ångfartyget RMS Titanic , vilken kolliderade med ett isberg under sin jungfruresa natten den 14– 15 april 1912 och sjönk, varvid Smith omkom.
